Home care is bought on trust and sold on adjectives. Certification replaces the adjectives with a published standard, an independent audit including observed visits, and a register anyone can check in ten seconds.

The standard

The five standard domains

The full criteria run to 63 pages and are free to download before you apply — most providers use them as an improvement checklist and never certify at all, which is a perfectly good outcome.

Clinical governance

Domain 1

A named responsible clinician, written supervision lines, case review at a stated frequency, and a governance record an auditor can read without being guided through it.

Scope and escalation

Domain 2

Documented limits on what may be delivered outside a facility, red-flag criteria, a reachable physician, and evidence that staff have escalated in practice rather than only in training.

Credentialing and education

Domain 3

Current licences verified at hire and annually, competency sign-off for every procedure offered, and continuing education hours completed — at the Field Institute or anywhere equivalent.

Consent, privacy and records

Domain 4

Informed consent that a patient could actually understand, secure records, a retention policy, and a complaints route that does not run through the person complained about.

Dignity and continuity

Domain 5

Care planned with the client rather than for them, continuity of staff where it is possible, and honest communication when it is not.

Levels

The three certification levels

Certified

€4,500 audit · €900 a year

The baseline. All five domains met at a single site or service line, verified by document review and an on-site audit including observed visits. Valid two years, with annual attestation.

Eligibility: Any licensed home-care, mobile-nursing or residence-care provider

Certified Plus

€6,200 audit · €1,400 a year

Baseline plus a declared specialism — infusion practice, post-procedure recovery, longevity diagnostics or dementia care — audited against the additional clinical requirements for that scope.

Eligibility: Certified providers with twelve months at standard

Centre of Excellence

By assessment

Certified Plus, plus a teaching role: the provider hosts Field Institute placements, contributes faculty, and submits outcome data twice a year. Fifteen sites hold this globally.

The audit

The audit process

    Self-assessment

    You score yourself against all five domains and send the evidence. Most applicants stop here for six months to fix what they find.

  2. 2

    Document review

    Policies, credential files, incident and complaint logs, and a sample of care records read remotely.

  3. 3

    On-site audit

    Two days: interviews with staff at every level, and observed visits with clients who have consented to an assessor being present.

  4. 4

    Decision

    Certified, certified with findings, or declined — in writing, with reasons. Declined applicants may reapply after six months.

  5. 5

    Surveillance

    Annual attestation, unannounced spot checks, and a direct complaints line that reaches us rather than the provider.

The public register

Search by provider, city or certificate number. Suspended and withdrawn certificates stay on the register with the date they lapsed — a register that only shows good news is not a register. The public copy lives at abc-worldwide.com/elixir.

The mark

Using the mark correctly

Certified providers receive the artwork in four variants. The mark must always carry the certificate number, and must always link to the register so a client can check it without taking anyone’s word.

  • Show the certificate number and the level exactly as issued
  • Link the mark to abc-worldwide.com/elixir
  • Never place it beside a claim of licensure, registration or clinical outcome
  • Apply it only to the service lines named on the certificate
  • Remove it within 14 days of lapse, suspension or withdrawal

What certification is not. It is not a licence, an insurance product, a guarantee of any clinical outcome, or an endorsement of prices. ELIXIR does not employ certified providers’ staff and is not a party to their care. Certification says that a published standard was met on the day it was audited, and that we check again.

Is this a licence?

No. Certification is a private standard held alongside whatever licence and registration your regulator requires, never instead of it. Any provider implying otherwise loses the mark.

Who audits?

A Field Institute faculty clinician who does not work for the provider and has no commercial interest in the outcome, accompanied by a second assessor for Centre of Excellence reviews.

What happens if a provider slips?

Findings are graded. Minor findings carry 90 days to correct; major findings suspend the mark immediately pending re-audit. Suspensions and withdrawals are published on the register with their date.

Can we be certified for one service line only?

Yes, and the certificate says so. A provider certified for home nursing but not for infusion may not display the mark on infusion material.

How long does it take?

Usually four to six months from application to decision, most of it spent on the provider’s side closing gaps found in self-assessment.

Do you certify individuals?

No. Individuals hold Field Institute course certificates; organisations hold certification. The two are deliberately separate.
